If you received a notice that it is time to complete your short survey:

 

BEFORE YOU START:

• This survey will take about 15 minutes to complete. Please set aside enough time to finish it in one sitting.

• If you are completing the survey over the Internet, please have your unique participant ID code and password that was sent to you. You will need this information in order to take the survey.

• Remember that after you complete the survey, we will be mailing you a $10 gift card to thank you for your time. So, please make sure to update your contact information.
